The referral form is complete when:
- Pages one and two are completed and signed by both a medical professional (MD, NP, RN, LCSW, etc.) and the client
- Pages three, four, and five are completed by the client (or someone on the client’s behalf)
- Proof of income must accompany each application; for example, photocopies of social security statements, pension statements or wage statements (income does not qualify or disqualify a person from service). Contact Client Services if you have questions or concerns about income verification.
Once we receive the referral form, the Client Services Coordinator will contact the referrer to confirm the receipt of the form. One of our Client Service staff will call the client to go through the intake process. During this call, which is typically 5-10 minutes, specific details about the client and our services will be reviewed. We take pride in meeting the needs of our clients and making sure our home-delivered meals make a positive impact on the quality of life for our clients.
Our goal is to improve our clients’ nutritional status, thereby strengthening their ability to fight their disease and, ultimately, improving their quality of life. We meet our vital goal through the following measurable primary objectives:
- To provide freshly prepared, nutrient dense, appetizing meals designed to meet the specific needs of adults and children coping with life-threatening illness. Meals can be modified, as necessary, to meet individual dietary restrictions (up to 70% of each day’s meals are modified).
- To provide up to the equivalent of three meals per day, five days per week, depending on need
- Each meal will provide 900-1000 calories with at least 15% from protein, 50% from carbohydrates, but no more than 30% from fat
- To base the level of service provided to each client on individual need as measured by the stage of the disease and availibility of resources
- To ensure our services reach medically underserved, financially disadvantaged, and minority populations
Our outcome goal is that after up to six months of service and annually thereafter, at least 70% of clients report, via written and phone survey, that their quality of life has maintained or improved over where it would have been without Project Angel Heart’s home-delivered meal service (according to six quality of life indicators, detailed here in the context of recent results). Among clients surveyed during the four quarters ending on 9/30/08 (the most recent survey results available):
- 93% reported feeling less stress around food acquisition and preparation
- 89% reported maintenance or positive change in energy level
- 84% reported maintenance or positive change in prescribed health regimen
- 82% reported maintenance or positive change in keeping healthcare appointments
- 84% reported maintenance or positive change in social/family relationships or obligations
- 83% reported maintenance or positive change in weight
