Starting this month, The Medical Center of Aurora’s registered dietitians will identify patients who have undergone bypass surgery, suffered a heart attack, or been diagnosed with COPD, and who are in need of nutritional support. As part of their prescribed health care plan, patients will receive medically tailored meals from Project Angel Heart for 30 days following their hospital stay. The meals, tailored to meet each individual’s medical and dietary needs, are designed to help them recover at home and keep from having to return to the hospital.
“We want to ensure that our patients are getting everything they need to help with their recovery after they’ve left the hospital,” said Brandon Mencini, TMCA’s Vice President of Business Development. “By connecting them with Project Angel Heart, we know they will be in good hands and will get the nutrition they need as they continue their recovery.”
