COVID-19 Volunteer Screening Questions

If you are volunteering in person, you will be asked the following screening questions when you arrive. If you can say “yes” to any of these questions, please notify a member of the volunteer resources team and do not come to your shift.

In the last 3 days, have you experienced any of the following symptoms:

  • Fever (100.4 degrees or higher)?
  • Persistent cough?
  • Shortness of breath?
  • Any health concern that affects your ability to volunteer or the health and safety of others?

Project Angel Heart asks all volunteers and staff to follow the public health and safety guidelines for the city and county in which they live. Have you had any instance in the last two weeks where you did not follow these guidelines? If you are unsure of the guidelines for your city and/or county, please reference the links below (if your county of residence is not listed, and you are unable to find the guidelines, please contact Mark Smith at msmith@projectangelheart.org):

Do you have any reason to believe you have been exposed to the coronavirus in the last 14 days?

Do you have any concerns about volunteering/working today?

Please note that we are also still asking anyone who has traveled internationally in the last two weeks to avoid visiting or volunteering at any of our facilities for at least 14 days following their return.
