Love good food? Love a good gathering? We’d love to have you as a Dining Out For Life Table Captain. Just invite a group of friends or colleagues to join you and dine together at one of our participating restaurants! You can also encourage your group to make additional contributions directly to Dining Out For Life!

As a table captain, you commit to:

• Reserve a table for your party at one of our participating restaurants on Thursday, April 23.
• Be the leader of your party, and tell them about Project Angel Heart!
• Bonus: Take a photo of your group dining out and tag us on social media (@projectangelheart)!

Complete the form below, telling us about your group and where you’ll dine, and we’ll give you a shout-out on social media. We might also swing by to take pictures of your crew and thank you all in person!
